Payment of high-cost inpatient medical care
Inpatients under 70 years of age who are incurring high medical costs may submit their Certificate of Eligibility for Ceiling-Amount Application in order to reduce the amount they have to pay. This amount is based on income.
We kindly request the following.
Please submit your Certificate of Eligibility for Ceiling-Amount Application at the time of admission.
The Certificate of Eligibility is normally valid from the first day of the month in which application is made. You will be charged the total amount of the hospitalization costs for the first month of your hospitalization in case your application for your Certificate of Eligibility for Ceiling-Amount Application is not made in a timely manner. Make sure to apply for it and submit it as soon as possible.
It is your responsibility to apply for and obtain a Certificate of Eligibility for Ceiling-Amount Application from your insurer (the issuer of your health insurance card). Following receipt of the application, the insurer will issue a Certificate of Eligibility for Ceiling-Amount Application, which will be based on your household income.
Procedure from the application for your Certificate of Eligibility for Ceiling-Amount Application to payment
* You will need to have your health insurance card and personal seal to apply.
* For details, please contact the issuer of your health insurance card.
Please visit your local city, ward, town, or village office if you are enrolled in the National Health Insurance program.
Please visit your local pension office or the insurance office of your union if you are enrolled in the Social Insurance program. Note 1: The Certificate of Eligibility for Ceiling-Amount Application does not cover costs for meals, pajamas, as well as other costs not covered by the health insurance (room upgrade, medical certificates, etc.). Note 2: Without a Certificate of Eligibility for Ceiling-Amount Application, you will be required to pay the amount that corresponds to the ratio (between 10 and 30%) as indicated on your health insurance card. * If you apply for your Certificate of Eligibility for Ceiling-Amount Application after you have made a payment, the amount exceeding the limit will be refunded to you from the insurer.
