Application for Medical Certificates and Attestation Forms

Life insurance company medical certificates

Applications cannot be processed during hospitalization. Please apply at the reception of the Inpatient Support Center (Counter 3) upon discharge (8:30 a.m. to 5:00 p.m. on weekdays).

Preparation for Hospital Admission

1. Documents and regular medication to bring on the day of hospitalization


2. Daily necessaries

Please bring the following items.

Eating utensils Chopsticks, spoon
Spare clothes Underwear, clothing items to wear over your pajamas
Towels 3 or 4 small towels, 2 or 3 bath towels
Personal care items Small washbowl, toothbrush, razor, comb, soap, shampoo
Slippers Slippers (non-slip sole) that cover the heel
Small bag A small bag to carry valuables such as your wallet when you need to leave your room
Other Tissue paper, garbage bags

3. Sleepwear

You can use your own pajamas. Hospital gowns are also available for ¥60 (tax included) per day.
Gowns are changed twice a week, or as necessary if soiled.

Please come directly to the inpatient ward at the appropriate time.

Daily Schedule

Meals and lights-out

Breakfast from 8:00 a.m.
Lunch from 12:00 p.m.
Dinner from 6:00 p.m.
Lights-out from 9:00 p.m.

Going out or staying out overnight

Obtaining more information about your medical treatment

Please ask your nurse if you would like to meet with your physician to talk about your illness, examination, or treatment. You can also ask your nurse about any other related matter.

Discharge

Discharge time

Discharge from the hospital is at 11 a.m.

Billing

The hospitalization bill will be sent to you at a later date. Please ask your nurse if you would like to know the rough estimate before you leave the hospital.

Counseling Desks

Counselors are available to assist you with any questions you may have. Do not hesitate to ask your nurse or attending physician.

Regional Collaboration Support Center

Medical social workers and nurses responsible for discharge are available to help you with daily matters related to your medical treatment after hospital discharge as well as with the practical details of the welfare system. The Regional Collaboration Support Center can answer questions about your hospitalization costs as well as other inquiries you may have.

Cancer Consultation Support Center

Professional counselors are available to answer questions patients and members of their families may have about cancer (treatment, etc.).

Palliative Care and Support Team

This team of specialists provide support to alleviate discomfort and painful symptoms associated with illness and treatment. Please ask your nurse or attending physician if you have any concerns.
